What is premature ejaculation (PE)?

Defined medically as a condition in which the male ejaculates before, at the time of, or immediately following insertion into the vagina. Put simply, if you can't delay your orgasm long enough to satisfy your partner most of the time or if you orgasm before you want to.

There are also extreme cases of PE where sexual intercourse does not take place at all, because the male partner ejaculates at the very start of foreplay even before insertion is attempted. Most men will have suffered from PE at some stage. We have had a case at the clinic where a 52 year old patient has had PE all his life after a bad experience with his first sexual partner. It is better to overcome PE as soon as possible, and this site provides a simple solution for this.

How do I increase the size of my penis?

Unfortunately the only method is quite extreme, surgery. It is expensive and should be a last resort. There are generally 3 types.

1. Fat transfer - this is where fat is taken from the thighs or lower abdomen and placed under the skin of the penis. This will increase the girth.

2. Suspensory ligament cut - A cut is made at the base of the penis and the suspensory ligament that holds the penis inside the body is released. This will add extra inches to your total length

3. Dermal draft - strips of skin with fat on are removed and placed under the skin of the penis.Men who have these forms of surgery often have to return a 2nd or 3rd time for further surgery until they are happy with the results. As with all surgery there are risks involved, like infection and scarring.

The main difference in treatment will be found in the strength of the dose you are prescribed. The treatment options include: Oral medication is effective in treating many men. After an examination, the physician will advise the patient as to whether he should pursue this option. Self-injection therapy involves inserting medication into the penis with a very fine needle and syringe. The medication is easily and painlessly self-injected before sex, creating a firm and sustained erection. For men with less severe erectile dysfunction (ED), injection therapy has a higher chance of ‘curing’ the condition so that the man will no longer require medication to obtain and maintain a firm erection. This option has a success rate of over 95% A vacuum device involves placing a hollow vacuum tube over the penis and inducing an erection with a pump, which draws the air out of the tube and the blood into the penis. The erection is maintained by placing a specially designed rubber band at the base of the penis. A urethral gel suppository involves gently inserting a tiny amount of gel into the urethra (the normal pathway for urine). The medication dissolves into the penile tissue, causing an erection.

How do medications such as Viagra, Cialis and Levitra work? What are the possible side effects?

Viagra, Cialis and Levitra are all part of a class of medications called PDE5 inhibitors. They all work in a similar way by preventing the breakdown of one of the chemicals in the penis that helps to relax blood vessels and enhance blood flow. More simply put, these drugs help to increase blood flow to the penis to produce longer-lasting, firmer erections. Sexual stimulation is required for these medications to work. Viagra, Cialis and Levitra have similar common side effect profiles with some differences. It is possible for a man to experience side effects with one of these drugs, and not with another. Some men don't experience any side effects. If the doctor determines that these medications are safe for you, the only way to really know which treatment will produce the least side effects is to try it. Generally, you should try the medication at least 3 or 4 times before determining whether or not it is the right treatment option for you.

I have just had prostate cancer surgery. Will I be able to function again?

You definitely have the potential to resume full sexual function. It may take some time before you know whether or not the surgery affected your sexual functioning, but don't wait, meet with an experienced doctor about these concerns. While some doctors may tell you to simply wait and see, other doctors experienced in sexual medicine might offer you options soon after surgery to help 'prevent' sexual problems from developing. If you discover that you do indeed have erectile dysfunction as a result of prostate surgery, successful treatment to help you achieve strong erections is certainly possible. In most cases the self injection therapy gives the best results. Other things to consider before you undergo prostate cancer surgery are: the type of surgery you are offered (there are nerve-sparing techniques available) and the experience of your surgeon. These factors will affect your erectile health post-surgery.
